Participant Rights - You have the following rights regarding your information:

§ To request an accounting of Geno.Me, Inc.’s disclosures of your information;
§ To request that we communicate with you in a certain way or at a certain location; and
§ To receive a copy of the full version of our Privacy Policy.
We may use and disclose medical information about you for the following purposes:
§ For functions necessary to run Geno.Me, Inc. and assure that our participants receive quality service;
§ To collect and issue payment for services; and
§ As required or permitted by law.
There are additional situations where we may disclose medical information about you without your authorization, such as:

§ In response to a court or administrative order, subpoena, warrant or similar process;
§ To law enforcement officials in certain limited circumstances;
§ To a coroner, medical examiner or funeral director; and
§ To organizations that handle organ, eye, or tissue procurement or transplantation.

OF SUMMARY NOTICE OF PRIVACY PRACTICES
Our Privacy Policy provides information about how we may use and disclose protected health information about you. The notice contains participant rights section describing your rights under the law. You have the right to review our Notice before agreeing to this Consent. The terms of our Notice may change. If we change our Notice, you may obtain a revised copy by contacting our office at support@yourgeno.me.
You have the right to request that we restrict how protected information about you is used or disclosed for treatment, payment, or health care operations. We are not required to agree to this restriction, but if we do, we shall honor that agreement.
By agreeing, you consent to our use and disclosure of protected health information about you for treatment, payment and health care operations. You have the right to revoke the Consent in writing, signed by you. However, such a revocation shall not affect any disclosures we have already made in reliance on your prior Consent. Geno.Me, Inc. provides this form to comply with the Health Insurance Portability and Accountability Act of 1996 (HIPPA).
The participant understands that:
• Protected health information may be disclosed or used for treatment, payment, or health care operations.
• Geno.Me, Inc. has a Privacy Policy, and that the participant has the opportunity to review this notice.
• Geno.Me, Inc. reserves the right to change the Privacy Policy.
• The participant has the right to request restrictions to the uses of their information but Geno.Me, Inc. does not have to agree to those restrictions.
• The participant may revoke this Consent in writing at any time and full disclosures will then cease.
• Geno.Me, Inc. may condition receipt of treatment upon the execution of this consent.
I have received a copy of the Summary Notice of Privacy Practices. I understand that I may also request a copy of the practice’s complete Privacy Policy if I so desire.
